A legal document in which one person (the principal) appoints another person (the agent) to make decisions related to health care. It applies when the principal is not able to make decisions for themselves. Instructions to the surrogate decision maker can be included in the document.

Relevance

This is necessary in the case where someone else needs to make decisions related to your care when you are unable. This should be completed well ahead of the time it is needed.
